Work Permits

Steps to Obtain a Work Permit

During the school year while distance learning through 9/11/2020.

Step 1: Get Forms and fill them out

  1. Get a copy of Intent to Employ a Minor form. It can be obtained from Mr. Hunt in room 154, googled, downloaded from the LAUSD website, or click link below to editable PDF. https://www.dir.ca.gov/dlse/dlseformB1-1.pdf
  2. Fill out the minor’s information, school information, and your counselor’s name.
  3. Bring form to your employer to be and have them complete the employer section including basic description, printed name, signed name, and dated.
  4. Bring form to parent or guardian for a signature.
  5. Attach a copy of your Social Security Card and one of the following proof of age:
    • student ID, birth certificate, passport, California ID or Driver's License

Step 2: Return Forms to Mr. Hunt

  1. Return form to either:
    • Mr. Hunt in room 154, Wednesdays or Fridays
      • 7:30 - 8:50am before school
      • 2:15 - 3:15pm after school
    • other days please place in box in main office and email rhunt1@lausd.net to let me know it was dropped off.
  2. Please park in the Walgrove parking lot for access to Walgrove building and room 154.

Step 3: Pick up your permit

  • Permit will be ready for pickup up within 48 hours after completed application is submitted, but only available for pickup Wednesdays and Fridays.
  • Possible to complete same day if turned in before school and picked up after school.

Work permit forms (Intent to Employ a Minor)

To maintain their Work Permit, Minors may be required to:

  • Maintain satisfactory school attendence
  • Minimize tardy periods and unexcused absences
  • Maintain school discipline standards
  • Meet all school academic standards
